remove expired session if refresh token missing - #1835
return 401 Unauthorized error
this avoids openid panic
Summary by CodeRabbit
Bug Fixes
Refresh requests with a missing or empty refresh token now clear the session and return an unauthorized error prompting the user to sign in again. The error also explains that the identity provider must issue a refresh token. When a refresh token is available, existing refresh behavior remains unchanged.

Walkthrough
The OAuth refresh flow checks for a nonempty refresh token before calling the OIDC client. If the token is missing or empty, the flow removes the session and returns an unauthorized error that directs the user to sign in again and request the offline_access scope.

A concurrent request may remain authorized after an expired session is removed. Resolve that authorization gap before merging unless the risk is explicitly accepted.

Revalidate authorization after session removal.
The new missing-refresh-token branch adds a session-removal path to an existing race. One request can compute an authorized auth_result, then another request can remove the same session. Because is_session_expired returns false for a missing session, the first request can still use its cached authorization result and reach the handler.
Revalidate the session and authorization after the refresh/removal path, before running the handler. The ordering flaw also affects the pre-existing refresh-failure removal path, so this PR broadens an existing authorization gap rather than introducing its underlying race.

Review comment at @src/handlers/http/middleware.rs at line 277:
Update the request flow around is_session_expired to revalidate that the session
still exists and authorization remains valid after any refresh or
session-removal path, before invoking the handler; do not rely on the earlier
cached auth_result when a concurrent request may have removed the session.
