docs: the plan service is named Medical Benefits - #18

From the API's spec 1.0.0-preview.12, the InsurancePlan service is named Medical Benefits, on docs.haau3.com and in the spec's tag. The README's list of services and its sample comment use that name, and the package keyword follows. Method names are unchanged: they come from the operations, not the tag. The API reference's pages for the service move with the tag to /api-reference/medical-benefits/, which api.md links once it is regenerated from that spec.
