Summary

Adds the release train 4 issue-triage record under devlog/_plan/260927_release_train_4/issue-triage/. It covers the decision and posted comment for each open issue owned by this lane (15, none closable yet) and for 21 large PRs (all held, none closed), plus the two focused fixes carried from them: #6098 (from #6079) and #6100 (from #5782). Move the completed unit out of _plan

This final outcome declares the lane done and records both carry PRs as merged, but the entire unit remains under devlog/_plan/ and includes _handoff.md instructions to push/open work that has already landed. Move the completed record to _fin and retire or finalize the stale handoff so a receiving maintainer does not repeat the publication steps; repository guidance reserves _plan for open units and requires closed units to be moved.
