Merge Risk: High · up to 40cee

The dashboard still contains material correctness and governance gaps, including non-persistent suppressions, inaccurate coverage and evidence completeness, stale evaluations, and insufficient override and audit validation.

Security architecture risk: Moderate · up to 40cee

The dashboard can display old results under a newly selected workspace and present temporary local exemptions as recorded governance actions. The API also accepts caller-asserted approval identities. These weaken report trust, although the inspected flows do not persist shared exemptions or modify source databases.

Retained concerns

  • Medium · security · observed: The endpoint newly converts caller-supplied suppressions and severity overrides into applied governance records, including an arbitrary authorized_actor. Its workspace dependency checks schema-read permission, while governance constructors only require a nonempty actor; the evaluator then describes overrides as authorized. This permits false approval attribution in server-generated reports. The base ignored these arrays. Exposure is report-local: existing rule customization, retained findings and the absence of a shared governance write substantially limit the impact.
  • Medium · security · observed: The new dashboard does not bind accepted reports to the current workspace/schema or latest request. Workspace changes retain the previous schema selection and report; late responses can overwrite newer results. Empty-schema and failure paths also retain the old report, and errors are displayed only when no report exists. Because the header uses the current workspace name, previously loaded schema evidence can be misattributed to another workspace. This is client-side ownership confusion, not an established bypass of server-side tenant isolation.
  • Medium · security · observed: The dashboard presents exemptions as recorded, expiring and auditable, but apply/revoke only edit React report state. The page discards justification and duration, creates a local suppression identifier, and sends no suppression configuration during reevaluation. Reevaluation or remount therefore loses the exemption; revocation likewise changes only the displayed result. This breaks the advertised governance ownership and audit lifecycle, without establishing a persistent policy change.
Security review details

Security Blast Radius

  • inferred — The demonstrated backend impact is bounded to generated reports for schemas the caller can address through the workspace resolver. Client ownership confusion can involve reports from multiple workspaces previously loaded in the same browser session. Shared policy persistence, source-database modification and cross-tenant server access are not established by these paths.

Security Findings and Attack Paths

  • observed — A caller can submit an override for a known finding fingerprint with another actor’s identifier. The new parser constructs the override, the evaluator applies its severity and emits an authorized-actor rationale, and the response includes the applied record. This establishes approval-provenance confusion; downstream acceptance as an authoritative audit decision remains unproven.

Trust Boundaries and Controls

  • observed — The existing workspace resolver validates workspace UUIDs and, when database and authenticated-user context are available, active membership and schema-read permission. Schema retrieval uses an exact workspace/schema tuple. Core suppression processing checks active status and expiry and retains suppressed findings in the report. These controls limit exposure but do not authenticate caller-supplied approval attribution.

Resilience and Maintainability Implications

  • inferred — Exemption repetition, reevaluation and recovery have no durable governance owner in the inspected flow. Local functional updates keep the displayed suppressed count synchronized, but they cannot provide approval provenance, expiry enforcement or recovery of the discarded justification.

172-216: Trivial · Maintainability & Code Quality · Quick win

Replace the hasattr and getattr guards with direct attribute access.

DiagnosticFinding is an internal model that the evaluator now enriches. Each hasattr(f.x, "value") guard turns a plain str field into None. A missing field also becomes None or False without any error.

The same applies to the getattr(report, ..., "1.0.0") defaults at lines 250-253. A version default hides a report that failed to carry its real policy or catalog version.

Access the fields directly. If a field can be absent, declare it Optional on the model. Then a model change fails loudly, and the response does not hide it.

Update the finding serialization block to access DiagnosticFinding fields
directly instead of using hasattr/getattr guards and fallback values, preserving
actual string and enum values. Declare fields Optional on the model only when
they may legitimately be absent, and replace the report version defaults with
direct access so missing required versions fail visibly.

309-310: Trivial · Stability & Availability · Quick win

Replace the wall-clock assertion with a non-flaky check.

assert duration < 1.5 depends on runner speed. Shared CI runners and the 3.12 and 3.13 matrix jobs can exceed the limit under load. The test then fails with no code change. Mark it as a benchmark and exclude it from the default run, or raise the limit well above the expected time. The correctness assertion on len(sorted_findings) can stay in a normal test.

Review comments at @apps/web/src/app/(app)/diagnostics/page.tsx:
- Around line 78-105: Stop passing handleApplySuppression and
handleRevokeSuppression to SuppressionControls until they persist changes
through an API, so its existing disabled-button behavior prevents users from
recording session-only suppressions as durable exemptions.

Review comments at @apps/web/src/components/diagnostics/SnapshotSelector.tsx:
- Around line 49-53: Connect SnapshotSelector to DiagnosticsOverview by loading
the workspace’s available snapshots and passing them as availableSnapshots.
Implement onSelectSnapshot to request and display the selected snapshot’s
report, while preserving the live-report path when no snapshot is selected.
- Around line 59-62: Update the onEvaluateLive flow to refresh the schema from
the database before evaluating it, so “Re-evaluate” reflects current database
changes; if refreshing is not supported there, change the action’s title to
describe evaluating the registered schema instead of claiming it uses the live
schema.

Review comments at @apps/web/src/lib/diagnostics/formatters.ts:
- Line 118: Update the explainability statistics in the formatter to read the
backend’s total_rules_considered and evaluated_rules_count fields instead of
total_rules_registered and rules_evaluated. Map both fields before deriving
rule-coverage statistics so valid live reports display the backend-provided
counts.

Review comments at @docs/audit/PHASE_8_5_SEVERITY_EVIDENCE_BASELINE.md:
- Line 41: Replace the three machine-specific Windows file links in the severity
evidence baseline with repository-relative links to enums.py,
evidence/models.py, and fingerprint.py. Keep the existing link labels and ensure
each relative target resolves from this document.

Review comments at @docs/diagnostics/SEVERITY_EVIDENCE_SECURITY.md:
- Around line 14-15: Update the suppression and effective-severity statements in
the security specification to describe authorized_actor as actor attribution
only; remove claims that diagnostics factories enforce workspace roles,
including the unsupported lead_engineer role.

Review comments at @packages/core/src/dera/diagnostics/evidence/validator.py:
- Around line 139-145: Update the known-object collection in the evidence
validator so it includes canonical id and object_id values for tables, columns,
indexes, primary keys, foreign keys, unique constraints, and check constraints,
plus node and edge IDs from resolve_graph(context). Preserve the existing
name-based checks so valid schema-backed evidence references are not marked
unresolved.

Review comments at
@packages/core/src/dera/diagnostics/rules/constraints/foreign_keys.py:
- Around line 95-100: Update the table index-building loop so schema-qualified
keys use the table’s namespace when schema_name is unavailable, while preserving
the existing schema fallback and lowercasing behavior. This ensures
referenced_schema lookups in the foreign-key rule can find tables declared with
namespace.

Minor comments:
Review comments at @apps/api/src/dera_api/schemas/reverse_engineering.py:
- Around line 509-514: Remove the unused suppressions and severity_overrides
fields from DiagnosticEvaluateRequest, since evaluate_schema_diagnostics does
not apply them. Regenerate docs/api/openapi.v1.json so the API schema no longer
advertises these unsupported request fields.

Review comments at @apps/web/src/app/(app)/diagnostics/page.tsx:
- Around line 235-236: Update the diagnostics header to use the selected
schema’s database_name for real schemas and the report’s source_engine for its
engine label, retaining the existing demo labels and using “Unknown” when either
value is unavailable. Add source_engine to the DiagnosticReport type so the
value can be accessed without a cast.
- Around line 65-70: Update loadDiagnostics to read the selected schema ID from
a ref and keep that ref synchronized when choosing an ID; remove
selectedSchemaId from its dependencies so the useEffect keyed to loadDiagnostics
does not trigger a duplicate evaluation on mount.

Review comments at @apps/web/src/components/diagnostics/CategorySummary.tsx:
- Line 56: Build displayedCategories from categoryCounts so the summary includes
every category present in the report, including foundational-rule findings. When
rendering categories without an entry in CATEGORY_META, use fallback
presentation metadata.

Review comments at @apps/web/src/components/diagnostics/DiagnosticsOverview.tsx:
- Line 64: Replace the stored selectedFinding object with its fingerprint, and
derive the selected finding from the current report.findings so the detail panel
reflects updated findings and clears when the finding is removed. Update the
selection and close handlers to set or clear the fingerprint.

Review comments at @apps/web/src/components/diagnostics/EvidenceInspector.tsx:
- Line 79: Update the table link in EvidenceInspector to include the encoded
affectedObjectId as the tableId path segment, matching the registered
`/app/tables/:tableId` route instead of passing it as a query parameter.

Review comments at @apps/web/src/components/diagnostics/EvidencePanel.tsx:
- Line 77: Update the expected-value rendering in EvidencePanel so arrays and
objects preserve their structure by rendering them as JSON, matching the
existing observed_value display; continue rendering primitive values as strings.

Review comments at @apps/web/src/components/diagnostics/FindingDetails.tsx:
- Line 96: Update the table link in FindingDetails to place the encoded
finding.object_id in the /app/tables path segment instead of a query parameter,
and apply the same path-segment pattern in EvidenceInspector using
affectedObjectId.

Review comments at @apps/web/src/components/diagnostics/FindingsExplorer.tsx:
- Around line 38-44: Update handlePrevPage and handleNextPage to use safePage
for their boundary checks and page updates, so each click navigates from the
clamped page when filters reduce totalPages.

Review comments at @apps/web/src/components/diagnostics/FindingsFilters.tsx:
- Around line 118-124: Update the category select in FindingsFilters so users
can filter findings in every category the backend emits, including
STRUCTURAL_INTEGRITY and METADATA_COMPLETENESS. Derive the options from
report.findings or reuse a shared category contract instead of hard-coding only
the current categories.

Review comments at @apps/web/src/lib/diagnostics/api.ts:
- Around line 177-188: Add credentials: "include" to the fetch options in the
diagnostics request so cross-origin requests send the session cookie. Keep the
existing method, headers, body, and cache settings unchanged.
- Around line 26-127: Update normalizeDiagnosticReport so missing, null, or
empty confidence and metadata_completeness values are not represented as
engine-verified states. Use the backend’s unknown value for
metadata_completeness; for confidence, preserve absence and adjust its type if
needed, since no unknown enum member exists. Keep supplied values consistent
with the backend enum casing.

Review comments at @apps/web/src/lib/diagnostics/formatters.ts:
- Line 123: Update the coverage calculation in the formatter using the count of
applicable rules as the denominator instead of total registered rules; keep
skipped applicable rules included in that count. Preserve the existing
evaluated-rule count and percentage calculation behavior.

Review comments at @apps/web/tests/e2e/diagnostics.spec.ts:
- Around line 4-11: Make the diagnostics E2E tests deterministic by establishing
authenticated state and stubbing or seeding the required schema and diagnostics
data before dashboard assertions; alternatively, click “View Verified Demo
Report” before asserting fixture-backed content. Update the “navigates to
diagnostics and displays overview metrics” test and the second diagnostics test
so neither relies on an unauthenticated, uninitialized page or preloaded
findings.

Review comments at @docs/audit/PHASE_8_COMPLETION.md:
- Line 60: Update the Phase 8.2 rule list in the Implementation entry to include
DERA-IDX-001 through DERA-IDX-004 and DERA-CON-001 through DERA-CON-005, using
the baseline’s structural-integrity description for DERA-CON-001. Also correct
the maintenance rule IDs in the audit record from DERA-MAINT-001 through
DERA-MAINT-004 to DERA-MNT-001 through DERA-MNT-008.

Review comments at @docs/diagnostics/COLUMN_PATTERNS.md:
- Line 29: Update the PostgreSQL / Supabase entry in the DERA-MNT-008 column
patterns documentation to list only text and bytea as evaluated LOB types;
remove json and jsonb.

Review comments at @docs/diagnostics/ENGINE_INDEX_SEMANTICS.md:
- Line 9: Update the MariaDB entry in the Expression / Functional row of the
index semantics table to match the Phase 8.2 baseline: MariaDB does not support
functional-index syntax and uses indexed virtual columns instead. Preserve the
other database entries and the row’s existing format.

Review comments at @docs/diagnostics/FINDING_IDENTITY.md:
- Around line 20-23: Update the documented evidence sort key in the canonical
payload snippet to use the serialized canonical dictionary as a final
tie-breaker after field_name and observed_value, and align the “Evidence Order
Invariance” text with that behavior.

Review comments at @docs/diagnostics/MAINTAINABILITY_OVERVIEW.md:
- Line 13: Update the `DERA-MNT-001` row in the maintainability overview table
to list `INFORMATIONAL` as its default severity, matching `TableWidthRule` and
the documented tiered severity behavior.

Review comments at @docs/diagnostics/MAINTAINABILITY_THRESHOLDS.md:
- Line 5: Remove the claim that threshold inputs are strictly validated during
initialization from the maintainability thresholds documentation;
`DiagnosticEngineConfig` and `TableWidthRule.evaluate` do not provide that
behavior. Keep the documented configurability claim unchanged.

Review comments at @docs/diagnostics/PRIMARY_KEY_DIAGNOSTICS.md:
- Line 12: In the Phase 8.2 description, remove the reference to “Per prompt
rule 38” and state directly that tables without primary keys are not classified
as errors because staging, logging, and read-only event-buffer tables can
legitimately omit them.

Review comments at @docs/diagnostics/PRIORITIZATION.md:
- Around line 11-43: Update the score formula and weight descriptions in
PRIORITIZATION.md to match compute_priority_score: severity.rank × 1000,
confidence.rank × 100, plus classification.priority_weight, with no scope term.
Replace the score-threshold ranks with derive_priority_rank’s
P1_CRITICAL–P5_INFORMATIONAL mapping, and update FINDINGS_EXPLORER.md to use
consistent rank names; document any UI mapping to the former names only if the
UI actually defines one.

Review comments at @docs/diagnostics/RECOMMENDATION_CATALOG.md:
- Line 27: Update the `REC-CON-001` action in the recommendation catalog to
describe empty primary key declarations, duplicate key columns, and missing
column references instead of nullability. Replace stale `DERA-MAINT-*`
references with the implemented `DERA-MNT-*` IDs in the catalog, classification
guide, and severity rationale.

Review comments at @docs/diagnostics/SUPPRESSION_GOVERNANCE.md:
- Around line 11-13: Update the suppression-field documentation to match the
contracts: use FINGERPRINT, OBJECT, and RULE with their canonical values, remove
GLOBAL, document fingerprint and scoped_object_id instead of target_identifier,
and describe DiagnosticFinding’s flat is_suppressed and suppression_id fields
instead of a nested suppression_state.

Review comments at @docs/diagnostics/UNRESOLVED_RELATIONSHIPS.md:
- Line 7: Update the REFERENCED_TABLE_NOT_FOUND severity documentation to
MEDIUM, matching the passing test and Phase 8.3 baseline; reconcile the
corresponding entries in RELATIONSHIP_SEVERITY.md and the Phase 8.3 acceptance
audit so they state the same default.

Review comments at @packages/contracts/src/index.ts:
- Line 122: Update DiagnosticSeverityLevel to include the API’s uppercase
severity values alongside the existing lowercase values, keeping the shared
contract aligned with the uppercase payloads handled by the web normalizer and
SeverityBadge.

Review comments at @packages/core/src/dera/diagnostics/evaluator.py:
- Around line 234-241: Validate matching overrides in the evaluator’s override
block with validate_severity_override, passing the override and classification
before changing the effective severity or recording the override. Apply and
record only accepted overrides; for rejected overrides, leave the existing
severity unchanged and append a rationale note with the rejection message.

Review comments at @packages/core/src/dera/diagnostics/evidence/validator.py:
- Around line 132-134: Update evidence type selection in the validator around
`_infer_evidence_type`: when an item’s `evidence_type` is absent or still
`CANONICAL_OBJECT_REFERENCE`, infer the type from `field_name` and
`observed_val`; otherwise preserve its explicit type.

Review comments at
@packages/core/src/dera/diagnostics/governance/suppressions.py:
- Around line 84-88: Update the expiry check in the visible suppression method
to parse `current_time` and `self.expires_at` with `datetime.fromisoformat` and
compare the resulting aware datetimes, treating naive values as UTC. Preserve
the existing behavior when no expiry is set.

Review comments at
@packages/core/src/dera/diagnostics/recommendations/catalog.py:
- Around line 134-175: Update the REC-CON-001, REC-CON-002, and REC-CON-003
catalog entries so their descriptions and remediation guidance accurately
reflect the checks mapped by DERA-CON-001, DERA-CON-002, and DERA-CON-003,
respectively, rather than recommending unrelated primary-key or foreign-key
changes. Make the corresponding classification comments consistent with those
checks as well.

Review comments at
@packages/core/src/dera/diagnostics/reports/explainability.py:
- Around line 77-81: Update the evaluation loop to compare ev.status directly
with the EvaluationStatus.NOT_APPLICABLE and EvaluationStatus.UNSUPPORTED
members, so messages populate skipped_reasons and unsupported_reasons. Import
EvaluationStatus from the project’s existing diagnostics enums module if it is
not already available.

Review comments at @packages/core/src/dera/diagnostics/rules/__init__.py:
- Around line 37-40: Update rules.ALL_RULES to include the Phase 8.3
relationship rules and Phase 8.4 maintainability rules alongside
ALL_FOUNDATIONAL_RULES and ALL_PHASE_8_2_RULES, reusing the existing aggregate
rule collections so imports from both diagnostics modules expose the same
complete set.

Review comments at
@packages/core/src/dera/diagnostics/rules/constraints/unique_constraints.py:
- Around line 182-187: Update the unique-constraint pair key built from name1
and name2 so it uses each constraint’s stable ID, avoiding name-based
deduplication and sorting None values. When a constraint has no name, use its
index position as the fallback identifier; preserve the existing seen_pairs
check and duplicate-skipping behavior.

Review comments at @packages/core/src/dera/diagnostics/serialization.py:
- Around line 68-71: Update stable_content in compute_report_content_hash to
include explainability, suppressions_applied, and severity_overrides_applied,
defaulting the applied-record fields to empty lists when absent, so all three
fields contribute to report_hash.

Review comments at @tests/diagnostics/test_evidence_traceability.py:
- Line 174: Format the nested dataclass declaration after the test docstring
with the required blank line, and remove the extra blank line at the end of the
file so it passes Ruff formatting.

Review comments at @tests/diagnostics/test_index_signatures.py:
- Line 234: Remove the extra trailing blank line at the end of
test_index_signatures.py, preserving the existing test content and formatting.

Review comments at @tests/diagnostics/test_phase_8_5_integration.py:
- Line 237: Wrap the overlong assertion comparing `deser_f` and `orig_f`
structured recommendation IDs across multiple lines to satisfy Ruff formatting,
without changing its behavior.

Nitpick comments:
Review comments at
@apps/api/src/dera_api/api/v1/endpoints/reverse_engineering/diagnostics.py:
- Around line 172-216: Update the finding serialization block to access
DiagnosticFinding fields directly instead of using hasattr/getattr guards and
fallback values, preserving actual string and enum values. Declare fields
Optional on the model only when they may legitimately be absent, and replace the
report version defaults with direct access so missing required versions fail
visibly.

Review comments at @tests/diagnostics/test_phase_8_5_integration.py:
- Around line 309-310: Update the timing check in this test so it is not a flaky
default-run gate: mark the performance test as a benchmark and exclude it from
the default test run, or raise the duration limit well above expected runtime.
Keep the correctness assertion on sorted_findings in the normal test.

apps/api/src/dera_api/api/v1/endpoints/reverse_engineering/diagnostics.py:110-145
Major · Data Integrity & Integration · Quick win

Wire the new request settings into the diagnostic evaluator.

When an authorized caller submits suppressions or severity_overrides, evaluate_schema_diagnostics drops both fields while constructing DiagnosticEngineConfig. The evaluator then receives empty tuples, so the endpoint returns an unmodified report.

Convert the request dictionaries into DiagnosticSuppression and SeverityOverride records, and pass those records in the config at apps/api/src/dera_api/api/v1/endpoints/reverse_engineering/diagnostics.py:129-136. Passing the dictionaries directly is not sufficient because the evaluator filters for those concrete record types.

apps/web/src/app/(app)/diagnostics/page.tsx:28-70
Minor · Stability & Availability · Quick win

Ignore stale loads and reset workspace-scoped state.

ws-default-local is only a client placeholder. The API rejects it as an invalid UUID, so its request cannot return schemas or a report. If that request finishes after the authenticated workspace load starts, its stale error and loading writes can expose the placeholder failure instead of the active workspace state. A workspace change can also reuse a schema ID from the previous workspace and receive a 404.

Track the latest request, clear state when the workspace changes, and use the stored schema ID only when it belongs to the new workspace.

Update the loadDiagnostics callback to ignore state updates from stale requests,
reset schemas, selection, and report when the workspace changes, and only reuse
targetSchemaId or selectedSchemaId if it exists in the new workspace’s schema
list. Apply the stale-request guard to success, error, and loading updates.

- Around line 175-176: Update both suppression and override entry handlers so
invalid entries produce an HTTP 422 response identifying the failing entry
instead of being silently skipped. Catch only expected parsing or construction
exceptions, allowing unrelated errors to propagate.
- Line 165: Bind authorized_actor in the diagnostics endpoint to the
authenticated current_user instead of trusting the request body's value, or
reject requests where the supplied actor does not match; ensure applied
governance records reflect only the authenticated identity.
- Around line 133-155: Update evaluate_schema_diagnostics to allow SCHEMA_READ
requests without governance inputs, but require an ADMIN or OWNER workspace role
whenever payload.suppressions or payload.severity_overrides is non-empty; reject
other roles before applying either collection.

Review comments at @packages/core/src/dera/diagnostics/evaluator.py:
- Around line 266-271: Validate each matched override with
validate_severity_override before applying it in the evaluator’s
overrides_by_fingerprint path, and apply its severity and record it only when
valid; otherwise leave it unapplied and include the validation reason. Also
validate overrides in the API endpoint that constructs them before accepting
them.
