Summary

This pull request implements DERA Phase 8.5: Severity, Evidence & Engineering Recommendations, establishing a unified, deterministic, evidence-based diagnostic intelligence architecture across all DERA diagnostic categories (Phase 8.1–8.4).

Phase 8.5 guarantees that every diagnostic finding explains:

  1. What issue was detected (unified finding contract, classification, confidence).
  2. Which rule detected it (stable rule ID and rule semantic version).
  3. Which database object is affected (verified canonical object identity).
  4. What evidence supports the finding (typed, validated, sanitized evidence records).
  5. Why the severity was assigned (conservative severity policy, human-readable rationale).
  6. Is the finding confirmed or a review candidate (categorical confidence terminology).
  7. Is the metadata complete (explicit completeness indicators).
  8. What engineering action is recommended (versioned, deterministic advisory recommendation catalog).
  9. What limitations affect the conclusion (explainability summary, engine capabilities).
  10. Can the finding be reproduced (cryptographic SHA-256 finding fingerprints, 100% deterministic evaluation).

Implementation Details

  • Base Commit SHA: e84f5ec4a905526d54bcbdb3e354fa7a151e07d7
  • Branch: feat/phase-8.5-severity-evidence-recommendations
  • Unified Finding Contract:
    • Enriched DiagnosticFinding and UnifiedDiagnosticFinding with classification, confidence, completeness, structured recommendation, lifecycle state, suppression state, priority score, and priority rank.
  • Conservative Severity Policy:
    • Deterministic severity rules (SEVERITY_POLICY_VERSION = "1.0.0").
    • Explicit guardrails: wide tables, missing FK indexes, cyclic dependencies, or naming discrepancies cannot escalate to CRITICAL.
    • Authorized overrides with actor, timestamp, and audit reason.
  • Evidence Traceability & Validation:
    • Strongly typed TypedDiagnosticEvidence (CANONICAL_OBJECT, COLUMN_REFERENCE, CONSTRAINT_REFERENCE, INDEX_REFERENCE, GRAPH_EDGE, METRIC_OBSERVATION, THRESHOLD_COMPARISON, etc.).
    • Object existence validation and secret minimization (scrubbing tokens, passwords, connection strings).
    • Categorical confidence levels (VERIFIED, CONDITIONAL, INCOMPLETE, UNSUPPORTED) instead of statistical guessing.
    • Completeness indicators (COMPLETE, PARTIAL, RESTRICTED, UNSUPPORTED, UNKNOWN).
  • Structured Recommendation Catalog:
    • Versioned catalog (RECOMMENDATION_CATALOG_VERSION = "1.0.0").
    • Safety validation prohibiting destructive SQL (DROP TABLE, TRUNCATE, CASCADE).
    • Purely advisory engineering recommendations with preconditions, suggested actions, risk considerations, and verification steps.
  • Prioritization & Ordering:
    • Explainable priority score formula: $S = (W_{\text{sev}} \times 1000) + (W_{\text{conf}} \times 100) + (W_{\text{cls}} \times 10) + W_{\text{scope}}$.
    • Deterministic multi-key stable sorting hierarchy (priority_score desc, severity desc, confidence desc, rule_id asc, object_id asc, fingerprint asc).
  • Suppression Governance & Lifecycle:
    • Scoped suppressions (FINDING_FINGERPRINT, OBJECT_ID, RULE, GLOBAL).
    • Active expiration handling, revocation, and append-only audit logging.
    • Non-silent suppression guarantee: suppressed findings remain visible in reports for compliance audits.
    • Lifecycle state machine: OPEN, ACKNOWLEDGED, SUPPRESSED, RESOLVED, REOPENED.
  • Explainable Reporting:
    • ReportExplainabilitySummary detailing total rules, evaluated rules, skipped rules, unsupported rules, insufficient metadata counts, and severity/classification breakdowns.
    • Subsystem version tracking and byte-for-byte serialization determinism.
  • API & UI Integration:
    • FastAPI diagnostic endpoint enriched with Phase 8.5 models, OpenAPI snapshot updated.
    • TypeScript contracts and accessible Next.js UI components (SeverityBadge, ClassificationBadge, EvidencePanel, RecommendationPanel, FindingDetailCard).

Testing & Quality Assurance

  • Unit, severity, evidence, recommendation, prioritization, suppression, and determinism tests pass.
  • OpenAPI live contract matching snapshot.
  • TypeScript typechecking passes (npm run typecheck).

evaluator so it generates recommendations before recording findings, overrides,

rejected rule’s evaluations as errors once, then skip all of that rule’s raw
findings in the main loop so no findings or governance records remain for it.
Add coverage for a rule with multiple findings where one recommendation is
rejected.

Change: Feature

Merge Risk: Moderate · up to 0f9d3

Diagnostic severity and governance results may be inaccurate or inconsistent. Resolve those issues before merging; the explainability status-comparison issue has been fixed.

Security architecture risk: Moderate · up to 0f9d3

The expanded diagnostic pipeline introduces request-level resource amplification and inconsistent recovery after recommendation-safety failures. Existing workspace checks and advisory-only recommendations limit the demonstrated exposure, but the changes warrant design-level review.

Retained concerns

  • Medium · security · inferred: Each finding now triggers a complete rebuild of known schema and graph identities. This introduces work proportional to findings multiplied by schema size, including columns. The default limits independently allow 100,000 objects and 10,000 findings, while the configured timeout is not enforced along the examined evaluation path. Because the async endpoint calls evaluation synchronously, a requester with access to a sufficiently large stored schema can impose substantial CPU work on the serving worker. The base evaluator did not perform this repeated enrichment scan. Workspace authorization and count caps constrain access and input size, but do not contain the new combined work; actual service-wide impact depends on deployment isolation.
  • Medium · reliability · observed: Recommendation-safety recovery is not atomic across findings, rule outcomes, and governance audit records. Overrides are recorded before recommendation validation. On rejection, the evaluator marks the rule errored and removes its assembled findings, but retains applied overrides and earlier suppression records. It also continues processing later findings from that same rule. A report can therefore describe an errored rule while emitting its later findings or orphaned governance records. This weakens the new safety gate's failure-containment and audit-consistency guarantees; it does not establish execution of prohibited SQL. Governance records are currently reachable through trusted in-process configuration, not forwarded from the examined HTTP request.
Security review details

Security Blast Radius

  • inferred — The demonstrated new availability exposure is synchronous computation in the API-serving worker. The route operates on a selected workspace's stored schema, but worker contention could affect unrelated requests sharing that worker. Cross-worker, cross-service, and environment-wide impact are not established without deployment evidence.

Security Findings and Attack Paths

  • inferred — A requester able to evaluate a large stored schema can select enabled rules and repeatedly trigger the new per-finding schema scans. This supports a CPU-exhaustion concern, not a measured outage or an anonymous attack claim. Membership checks, object limits, and finding limits are relevant countercontrols, but do not impose a combined enrichment-work budget.

Trust Boundaries and Controls

  • observed — Workspace resolution validates the workspace identifier and performs active-workspace, membership, and SCHEMA_READ checks when both database and current-user dependencies are available. This conditional enforcement predates the PR. The examined change does not establish a new tenant-boundary bypass, and production dependency availability remains unverified.
  • observed — Core governance records require explanatory reasons and nonempty actor identifiers, but those checks do not authenticate an actor. The examined HTTP handler does not forward governance records, so these models currently rely on their in-process caller's authority rather than demonstrating an externally reachable privilege transition.

Resilience and Maintainability Implications

  • observed — The recommendation-safety test exercises one rejected finding without governance state. The implementation's multi-finding recovery can leave errored-rule findings and orphaned applied-governance records, weakening consistency between the safety outcome and the audit report.

Hardening Proposals

  • proposed — Before enabling the advertised HTTP governance inputs, define an explicit authority boundary that binds actor identity and suppression or override scope to the authenticated workspace context. Nonempty actor strings should remain metadata validation, not authorization evidence.

Replace the open additionalProperties: true objects with typed schemas.

suppressions and severity_overrides items accept any object, so schema validation cannot reject malformed records. The same applies to structured_recommendation, explainability, suppressions_applied and severity_overrides_applied. Typed Pydantic models in reverse_engineering.py would generate closed schemas (extra="forbid"). That pins the contract the web layer already assumes (StructuredRecommendationItem, ReportExplainabilitySummaryItem).

Based on learnings: "Flag OpenAPI object schemas without additionalProperties: false, unless arbitrary extra properties are intentionally allowed".

consumed by StructuralCouplingRule and LobConcentrationRule, and correct the

stable sort hierarchy in the documentation to match the implementations in
ranking.py and ordering.py. Document the score as severity rank × 1000 plus
confidence rank × 100 plus classification priority weight, derive P1_CRITICAL
through P5_INFORMATIONAL from severity only, and list sorting by severity,
confidence, classification weight, object_id, rule_id, then fingerprint.

Review comments at @docs/diagnostics/SEVERITY_EVIDENCE_SECURITY.md:

Review comments at @packages/core/src/dera/diagnostics/evaluator.py:
- Around line 234-241: Update the override handling in the evaluator to call
validate_severity_override with the override and classification, and apply it
only when validation succeeds and override.original_severity matches
raw_f.severity. Otherwise, leave the finding’s severity unchanged and do not
record the override as applied.

Review comments at @packages/core/src/dera/diagnostics/evidence/validator.py:
- Around line 76-108: Update known_objects construction to include canonical
schema IDs, not just object_id: collect each table and column id, plus IDs for
indexes, foreign keys, unique and check constraints, and primary keys when
present. In the context.graph.nodes loop, add each node’s object_id as a string
rather than the TableNode object itself, and include graph edge object IDs as
strings so valid evidence resolves.

Review comments at

- Around line 84-88: Update the expiration check that uses self.expires_at and
current_time to parse both timestamps as aware datetimes and compare their
instants, treating Z and UTC offsets correctly and assigning UTC to any naive
timestamp before comparison.

Review comments at @packages/core/src/dera/diagnostics/rules/__init__.py:
- Around line 37-40: The integration test uses the narrower rules aggregate and
therefore omits Phase 8.3 and 8.4 rules; update its import to use the
package-root ALL_RULES, and avoid redefining ALL_RULES in the narrower rules
module.

Review comments at

- Around line 141-149: Update the `idx_cols` construction in the foreign-key
index coverage check to preserve each index term’s position, using a placeholder
for expression or unnamed terms so they cannot shift later columns into the
leading-key position. Normalize index and foreign-key column names
case-insensitively before comparing their prefixes, while preserving the
existing fallback for indexes without terms.

Review comments at @packages/core/src/dera/diagnostics/serialization.py:
- Around line 64-67: Update compute_report_content_hash so stable_content
includes explainability, suppressions_applied, and severity_overrides_applied
from report_dict; default the two governance lists to empty lists so changes to
these fields are covered by the content hash.
- Around line 106-154: Update the deserialization flow that builds
`DiagnosticFinding` so it restores `structured_recommendation`, preserves
`finding_id` instead of recomputing it, and reconstructs each
`DiagnosticEvidence` with its `evidence_type`, `completeness`, and `provenance`.
Add a round-trip test asserting that the hash recomputed from the deserialized
report’s canonical dictionary matches its stored `report_hash`.

Isolate recommendation-safety failures per rule.

InvalidRelationshipMappingRule inserts the foreign-key name into its raw recommendation, but DERA-SCHEMA-003 has no catalog entry. A reachable foreign key named DROP TABLE with an invalid column mapping can therefore fail the fallback safety check. The exception runs outside the evaluator's rule error boundary, and evaluate_diagnostics does not catch it. One malformed finding can abort the complete diagnostics report instead of marking that rule as EVALUATION_ERROR and continuing with the remaining rules. Preserve the safety rejection, but handle it in the per-rule post-processing path and keep the report counts consistent.

Fix the status comparisons. skipped_reasons and unsupported_reasons are always empty.

EvaluationStatus values are uppercase: "NOT_APPLICABLE" and "UNSUPPORTED". Line 78 compares ev.status.value to lowercase "not_applicable". Line 80 compares it to lowercase "unsupported". Neither comparison can match. Every report therefore has empty skipped_reasons and unsupported_reasons, even when rules were skipped or unsupported. This breaks a core explainability output of this PR.

Compare against the enum members instead of string literals. Add a test that builds a NOT_APPLICABLE evaluation and an UNSUPPORTED evaluation, each with a message. The test then asserts that both messages appear in the summary.

Include scope in priority scoring.

The reachable evaluator and both finding-model fallbacks compute priority from severity, confidence, and classification only. The Phase 8.5 contract also requires W_scope, with different weights for schema/global, table, and column/index/constraint findings. Otherwise, otherwise-identical findings at different scopes receive the same priority score and cannot be ordered by the documented scope priority. Add an explicit scope value to the scoring path, include its documented weight, and cover schema-wide versus column-scoped findings in the tests.

A safety rejection leaves a partly processed rule in the report.

The except RecommendationSafetyViolationError branch handles only the finding that raised the error. This causes two inconsistencies.

  1. Later findings of the same rule are not skipped. The branch marks raw_f.rule_id as errored and sets its evaluations to EVALUATION_ERROR with findings=(). The loop then continues with the next raw_f of that rule. If its recommendation is safe, the evaluator appends it to unified_findings. The report then lists a finding from a rule that coverage.errored_rules and rule_evaluations mark as errored. The recommendation text is per finding, so one unsafe finding does not mean the rest are unsafe.
  2. Stale governance records remain. Earlier findings of the rule are removed from unified_findings. Their entries in applied_overrides (recorded before this step) and applied_suppressions (recorded earlier in the same loop) stay in the report. These entries reference findings that the report no longer contains.

test_evaluator_isolates_recommendation_safety_violation uses one finding. It does not cover either case.

Run recommendation generation in a pre-pass before any finding is processed. Collect the rejected rule IDs. Then mark those rules errored once, and skip all of their raw findings in the main loop. Override and suppression records are then never created for dropped findings.

Add this before step 6, outside the selected range:

recs: dict[int, Any] = {}
rejected: dict[str, RecommendationSafetyViolationError] = {}
for idx, raw_f in enumerate(raw_findings):
    if raw_f.rule_id in rejected:
        continue
    try:
        recs[idx] = generate_structured_recommendation(raw_f)
    except RecommendationSafetyViolationError as rec_err:
        rejected[raw_f.rule_id] = rec_err

for rule_id_rejected, rec_err in rejected.items():
    if rule_id_rejected not in errored_rule_ids:
        errored_rule_ids.append(rule_id_rejected)
    for idx, ev in enumerate(evaluations):
        if ev.rule_id == rule_id_rejected:
            evaluations[idx] = RuleEvaluation(
                rule_id=ev.rule_id,
                rule_version=ev.rule_version,
                status=EvaluationStatus.EVALUATION_ERROR,
                findings=(),
                execution_duration_ms=ev.execution_duration_ms,
                message=f"Recommendation safety rejected: {rec_err}",
                error_details=str(rec_err),
            )

Then change the main loop to for idx, raw_f in enumerate(raw_findings):. Add if raw_f.rule_id in rejected: continue at its top. Replace the try/except block with these lines:

structured_rec = recs[idx]
rec_text = structured_rec.suggested_action if structured_rec else raw_f.recommendation

Add a test with two findings from one rule where only the first is unsafe. Assert that the report has zero findings for that rule.
