Implements DERA Phase 8.4 — Schema Maintainability Diagnostics.
This PR extends the deterministic diagnostic engine to audit database schema maintainability, structural complexity, naming consistency, namespace organization, table width, column composition patterns, and relationship coupling across canonical schema metadata without executing customer SQL or performing database mutations.
All rules operate strictly without AI/LLM/ML models or APIs, delivering explainable findings with verifiable evidence, deterministic ordering, and stable SHA-256 fingerprints.
Strict Non-AI Policy: Verified zero generative AI, LLM, or external inference APIs are introduced.
Database Safety: Verified all database connector operations enforce read-only introspection without data modification.
Credential Protection: Confirmed that no production credentials, passwords, or .env secrets are committed.
Least Privilege: Validated that permissions and access boundaries are minimized.
No Security Impact: Change does not affect database connections, security boundaries, or credentials.

Summary
Shared diagnostic models and analysis helpers packages/core/src/dera/diagnostics/context.py, packages/core/src/dera/diagnostics/metrics/*, packages/core/src/dera/diagnostics/rules/shared/*, tests/diagnostics/test_graph_helpers.py
DiagnosticContext.graph now uses ForeignKeyGraph. New shared metric models and calculators cover schema, table, namespace, naming, and coupling data. New graph helpers resolve graphs, dependencies, tables, and namespaces, and add reverse reachability and edge grouping.
Relationship and dependency diagnostics packages/core/src/dera/diagnostics/rules/relationships/*, docs/audit/PHASE_8_3_*, docs/diagnostics/{RELATIONSHIP_*,GRAPH_INTEGRITY,UNRESOLVED_RELATIONSHIPS,COMPOSITE_FK_VALIDATION,CROSS_NAMESPACE_DEPENDENCIES,CYCLIC_DEPENDENCIES,DEPENDENCY_*,SCC_ANALYSIS,STRUCTURAL_IMPACT,FUTURE_NOSQL_RELATIONSHIPS}, tests/diagnostics/test_{relationship,dependency}_*.py
Adds four relationship rules and four dependency rules, with shared catalog wiring, documentation, baseline and acceptance audits, direct rule tests, and permutation-based determinism tests.
Maintainability metrics and diagnostics packages/core/src/dera/diagnostics/rules/maintainability/*, docs/audit/PHASE_8_4_*, docs/diagnostics/{MAINTAINABILITY_*,COLUMN_PATTERNS,NAMING_CONVENTIONS,IDENTIFIER_SEMANTICS,NAMESPACE_ORGANIZATION,SCHEMA_COMPLEXITY,STRUCTURAL_COUPLING,TABLE_WIDTH,DEPENDENCY_COMPLEXITY,FUTURE_NOSQL_MAINTAINABILITY}, tests/diagnostics/test_maintainability_*.py
Adds eight maintainability rules for width, nullability, naming, reserved words, namespace imbalance, coupling, cyclic topology, and LOB concentration, with metric support, documentation, audits, rule tests, and determinism coverage.
Aggregate rule registration and package exports packages/core/src/dera/diagnostics/__init__.py, docs/audit/PR_PHASE_8_4_BODY.md
The package now imports, aggregates, and exports the Phase 8.3 and Phase 8.4 rule collections and classes. The Phase 8.4 PR body records the rule list, metrics modules, test claims, and checklist text.
Constraint and index diagnostic fixes packages/core/src/dera/diagnostics/rules/constraints/foreign_keys.py, packages/core/src/dera/diagnostics/rules/shared/index_signatures.py, tests/diagnostics/test_constraint_rules.py, tests/diagnostics/test_index_signatures.py
Schema-qualified foreign-key lookup now checks the qualified key directly before the unqualified fallback. Index predicate normalization now preserves literal case and internal whitespace, and unique narrow indexes are no longer treated as leftmost-prefix duplicates of wider unique indexes.

Merge Risk: Moderate · previous review 9fc0e
Several earlier concerns are still open: the API rule registration check, the documented checks that some rules do not implement, resource limits not enforced on all paths, and the formatting failure in the index-signature tests. One new edge case can make the duplicate-index diagnostic report two partial indexes as duplicates when their predicates differ only in whitespace inside double-quoted identifiers. Resolve these before merging or explicitly accept them.
Security architecture risk: Low · up to 9fc0e
The expansion remains an explicitly invoked diagnostics feature, with no demonstrated new database-write authority or external exposure. One resource-isolation concern remains: the new analysis paths can exceed a caller’s configured diagnostic limits, although separate internal limits constrain their work.
Retained concerns
Medium · security · observed: The new derived-analysis path does not preserve the caller’s diagnostic resource budget. With context.graph absent, the evaluator skips its edge-limit check, while graph construction and dependency analysis use independent defaults rather than DiagnosticResourceLimits. Multiple added rules can repeat this construction and analysis. Metadata exceeding a caller’s stricter budget can therefore consume resources in the executing process instead of being rejected at that boundary. Internal ceilings limit the work; attacker reachability and cross-tenant impact are not established.
Security review details
Security Blast Radius
inferred — The supported resource-isolation concern affects the process evaluating the added rules. An attacker would need influence over supplied schema or graph metadata and a way to trigger evaluation. The inspected evidence does not establish an unauthenticated endpoint, shared worker, tenant-wide exposure, or downstream privilege gain.
Security Findings and Attack Paths
inferred — A schema-only evaluation can pass the evaluator’s initial checks and then derive a graph exceeding a caller’s stricter max_edges setting. New consumers can repeat the associated construction and analysis. This supports an availability concern under hostile metadata, not a demonstrated external denial-of-service exploit.
Trust Boundaries and Controls
observed — Rule-selection authority remains explicit through the caller-provided iterable or registry registration. The added catalog entries do not themselves establish a new authentication or authorization boundary.
Resilience and Maintainability Implications
observed — Separate graph and dependency ceilings, bounded reverse reachability, and evaluator error isolation constrain failures. Fresh builders and analyzers avoid introducing shared mutable analysis ownership. These controls do not enforce one aggregate diagnostic work budget across repeated rule evaluations.
Hardening Proposals
proposed — Resolve and validate derived inputs once per evaluation, propagate the caller’s applicable limits into construction and analysis, and preserve resource-limit failures as distinct outcomes. This would align supplied and derived inputs under one failure-containment contract.

Empty-graph PASS branch is unreachable when dependencies are not preset.
resolve_dependencies returns None when context.dependencies is None and the graph has zero nodes. For an empty schema, deps is None is therefore true, and the rule returns INSUFFICIENT_METADATA at Line 65. The zero-node PASS branch at Line 75 never runs in that case.
RelationshipGraphIntegrityRule returns PASS for the same input. The status for an empty schema is therefore inconsistent across the Phase 8.3 rules.
DependencyDepthRule, DependencyConcentrationRule, and StructuralImpactRadiusRule have the same ordering. They have no zero-node check at all.
Check for zero nodes before the deps is None check. Apply the same change to the other three dependency rules, or move it into a shared helper.

@packages/core/src/dera/diagnostics/rules/relationships/cyclic_dependencies.py
around lines 65 - 83:
In the dependency rules, check whether the relationship graph has zero nodes
before treating missing dependency results as insufficient metadata, so empty
schemas return PASS even when dependencies are unset. Apply this ordering in
CyclicDependenciesRule, DependencyDepthRule, DependencyConcentrationRule, and
StructuralImpactRadiusRule, preserving INSUFFICIENT_METADATA for non-empty
graphs without dependency results.
Use real GraphCapabilityProfile field names in the test fixtures.
GraphCapabilityProfile defines supports_composite_keys, supports_deferrable and supports_cross_schema_fk. This fixture passes supports_foreign_keys, supports_composite_fks, supports_deferrable_fks and supports_cross_schema_fks. The model only sets frozen=True, so Pydantic's default extra="ignore" drops these kwargs silently.
The test therefore runs with default capabilities. supports_cross_schema_fk is False even though the fixture intends True. The same wrong kwargs appear in test_relationship_rules.py, test_relationship_determinism.py, test_dependency_rules.py, test_maintainability_rules.py, test_maintainability_determinism.py and test_maintainability_metrics.py. A future rule that reads these capabilities would see values different from the ones the tests state.

Update the GraphCapabilityProfile fixture in test_graph_helpers.py and the
corresponding fixtures in the other affected test modules to use the model’s
actual fields: supports_composite_keys, supports_deferrable, and
supports_cross_schema_fk. Remove the unsupported keyword arguments so the
configured capabilities are applied rather than silently ignored.
12-12: Trivial · Maintainability & Code Quality · Low value
Remove the metrics-to-rules import dependency.
coupling_metrics.py imports get_table_node_map from dera.diagnostics.rules.shared.graph_helpers. The maintainability rules import dera.diagnostics.metrics, so the two packages now depend on each other. Importing dera.diagnostics.metrics first can load rules/__init__.py while coupling_metrics is only partly initialized. structural_coupling.py then fails with an ImportError on compute_all_coupling_metrics. The current import order avoids this, but any reorder can break it.
The helper is one line. Inline it here, or move it to a module that does not depend on rules.
- node_map = get_table_node_map(graph)+ node_map = {str(n.object_id): n for n in graph.nodes}

Avoid rebuilding the graph in resolve_dependencies.
The rules call resolve_graph(context) and then resolve_dependencies(context). The second call runs resolve_graph again. When context.graph is None, ForeignKeyGraphBuilder.from_canonical therefore runs twice per rule. This happens for each of the rules that use both helpers.
Add an optional graph parameter to resolve_dependencies. Use it when the caller already holds a resolved graph.

@packages/core/src/dera/diagnostics/rules/shared/graph_helpers.py at line 65:
Update resolve_dependencies to accept an optional graph and resolve one only
when the caller does not provide it. In rules that already call
resolve_graph(context), pass that graph to resolve_dependencies so
ForeignKeyGraphBuilder.from_canonical is not invoked twice.
Do not hide graph-build failures behind a bare except Exception.
When ForeignKeyGraphBuilder.from_canonical raises, resolve_graph returns None. Examples of such errors are GraphBuilderError for a duplicate ObjectId and GraphResourceLimitError for a large schema. Every caller then reports INSUFFICIENT_METADATA with the message "No relationship graph or canonical schema available." The real cause is lost. The schema failure at Line 38 also skips the snapshot fallback.
Also, Line 41 reads context.snapshot.schema only. resolve_tables and resolve_namespaces read schema_representation first. A snapshot that exposes only schema_representation gets no graph but still gets tables.
Log the exception at debug or warning level. Narrow the exception type if possible. Align the snapshot attribute lookup with the other helpers.

33 - 38:
Update resolve_graph to avoid silently converting graph-build failures to None:
log the exception, catch only expected graph-building errors such as
GraphBuilderError and GraphResourceLimitError where possible, and continue to
the snapshot fallback. Read the snapshot schema using the same
schema_representation-first lookup as resolve_tables and resolve_namespaces,
while preserving the existing schema fallback.

- Around line 67-69: Remove the unused resolve_dependencies call and
dependency-result guard from the rule’s evaluation flow; the bounded traversal
uses only graph. Keep the missing-graph check based solely on
resolve_graph(context), so dependency-analysis failures and empty graphs do not
prevent the traversal from determining the result.

only outside double-quoted identifiers, preserving repeated whitespace inside
them; tokenize quoted identifiers before normalization or retain the stripped
predicate when quoting cannot be handled safely. Ensure predicates with distinct
quoted identifier whitespace produce distinct signatures used by
DuplicateIndexesRule.

Summary
Implements DERA Phase 8.4 — Schema Maintainability Diagnostics.
This PR extends the deterministic diagnostic engine to audit database schema maintainability, structural complexity, naming consistency, namespace organization, table width, column composition patterns, and relationship coupling across canonical schema metadata without executing customer SQL or performing database mutations.
All rules operate strictly without AI/LLM/ML models or APIs, delivering explainable findings with verifiable evidence, deterministic ordering, and stable SHA-256 fingerprints.
