feat(diagnostics): implement Phase 8.2 index and constraint analysis - #62
Implements Phase 8.2: Index & Constraint Diagnostics for DERA (Database Engineering & Reverse Analysis).
Extends the deterministic Phase 8.1 schema diagnostic framework with 9 production static analysis rules covering indexes and constraints:
DERA-IDX-001: Exact Duplicate Index Definition
DERA-IDX-002: Potentially Redundant B-Tree Index (Leftmost Prefix Analysis)
DERA-IDX-003: Foreign Key Supporting Index Coverage
Zero AI/LLM/ML: Completely deterministic rule evaluation, hash-based signature matching, and set algebra.
Strictly Read-Only: Evaluates in-memory canonical schema representations and snapshots with zero source database mutations.
Engine-Aware Semantics: Native engine support for PostgreSQL, Supabase, SQLite, MySQL, and MariaDB (including InnoDB auto-indexing exclusion for foreign keys).
Constraint Ownership: Distinguishes secondary indexes from primary key / unique constraint backing indexes to protect constraint integrity.
Evidence Model: Structured, sanitized DiagnosticEvidence payloads and deterministic SHA-256 fingerprints.

Summary by CodeRabbit
New Features
Added database diagnostics for duplicate and potentially redundant indexes, missing foreign-key index coverage, and indexes that reference nonexistent columns.
Added checks for primary keys, foreign keys, unique constraints, and CHECK constraints, including missing or inconsistent column references.
Diagnostics account for database-engine differences and identify when a check does not apply.
Documentation
Added guidance on diagnostic findings, severity, engine-specific behavior, performance, and safe analysis.
Adds nine index and constraint diagnostic rules, shared comparison and engine-capability helpers, and tests and documentation. The rule registries now include these rules, and the reverse-engineering service registers ALL_RULES.

Summary
Shared signatures and engine capabilities packages/core/src/dera/diagnostics/rules/shared/*, tests/diagnostics/test_index_signatures.py, docs/diagnostics/INDEX_SIGNATURES.md, docs/diagnostics/ENGINE_INDEX_SEMANTICS.md, docs/audit/PHASE_8_2_INDEX_CONSTRAINT_BASELINE.md
Adds normalized engine capability checks, constraint comparison helpers, canonical index signatures, and index ownership checks. Tests cover signature comparisons and engine capabilities.
Index diagnostics packages/core/src/dera/diagnostics/rules/indexes/*, tests/diagnostics/test_index_rules.py, docs/diagnostics/DUPLICATE_INDEXES.md, docs/diagnostics/REDUNDANT_INDEXES.md, docs/diagnostics/FK_INDEX_COVERAGE.md
Adds four rules to detect duplicate indexes, potential B-tree prefix redundancy, missing foreign-key index coverage, and index references to missing columns.
Adds five rules that check primary-key structure, foreign-key mappings, unique constraints, CHECK-constraint metadata, and constraint column references.
Rule registration and Phase 8.2 records packages/core/src/dera/diagnostics/__init__.py, packages/core/src/dera/diagnostics/rules/__init__.py, apps/api/src/dera_api/api/v1/endpoints/reverse_engineering/service.py, docs/audit/PHASE_8_2_INDEX_CONSTRAINT_*.md, docs/diagnostics/*
Adds aggregate rule registries and switches the reverse-engineering service to ALL_RULES. Adds Phase 8.2 audit records and reference material on diagnostic evidence, performance, security, severity, testing, and planned NoSQL indexing.
The new index and constraint diagnostics can produce misleading findings. They can recommend dropping an index that enforces uniqueness, report foreign keys as covered when they are not, and run even when a request disabled them. Fix these before merging; the rules only read schema metadata, so the effect is on diagnostic accuracy rather than data.
Security architecture risk: Low · up to 2878e
The new checks expand diagnostic results through existing read-only, workspace-authorized operations. No new database mutation authority or cross-workspace access path was established. Rule-selection and metadata-gating limitations affect the expanded catalog, so diagnostic output should not be treated as authority to modify indexes automatically.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — The established expansion is diagnostic processing and output for schemas reachable through the existing workspace boundary. The inspected registration change does not confer source-database write privileges or establish access to another workspace’s schema.
Trust Boundaries and Controls
observed — The evaluation route resolves an authorized workspace before calling the service. Workspace membership and SCHEMA_READ checks remain separate from rule applicability, and schema lookup uses both workspace and schema identifiers.
Hardening Proposals
proposed — Enforce caller rule selections and reconcile capability-profile propagation with the applicability interface before treating the expanded report as an authoritative coverage statement. These are diagnostic-control improvements, not demonstrated authorization fixes.
proposed — Keep query-prefix overlap distinct from safe constraint replacement: UNIQUE(a,b) does not preserve UNIQUE(a). Exclude narrower unique indexes from removal-oriented redundancy advice, or explicitly preserve that invariant in the output, before introducing any automated remediation consumer.

Add API tests for the evaluation endpoint and the Phase 8.2 rules.
The tests cover only the two rule-catalog routes. They assert DERA-SCHEMA-001 to 005 and omit the nine DERA-IDX-* and DERA-CON-* rules that this PR adds. If ALL_RULES loses those rules, the test still passes.
POST /schemas/{schema_id}/diagnostics has no test. The following cases need coverage:
A 404 for an unknown schema.
Workspace isolation, where a schema in workspace A returns 404 for workspace B.
A missing or malformed X-Workspace-Id header.
enabled_rule_ids and disabled_rule_ids filtering.
The response shape of the report.

Fix the capability lookup: SchemaCapabilityProfile has no get_state.
SchemaCapabilityProfile (packages/core/src/dera/reverse_engineering/capabilities.py:54-84) exposes features: dict[str, CapabilityState] and is_feature_supported(). It does not define get_state.
When a real profile is set on DiagnosticContext.capabilities, line 43 raises AttributeError for the first rule that declares required_capabilities. Most Phase 8.1 and 8.2 rules declare them.
DiagnosticEvaluator.evaluate calls check_rule_applicability outside its try block (evaluator.py line 100). The exception therefore aborts the entire evaluation. This breaks the documented error-isolation guarantee.
Read from features. Treat a missing key as unknown, not as unsupported. Also move the applicability call inside the evaluator's isolation block.

Apply DiagnosticEngineConfig rule selection and timeout_seconds in DiagnosticEvaluator.evaluate.
evaluate reads only context.config.limits. It hashes the full configuration into config_fingerprint, but it never applies enabled_rule_ids, disabled_rule_ids, included_categories, or excluded_categories. The API passes every registered rule and does not call filter_rules. A rule disabled in the request still runs and can report findings, while the fingerprint still records that disable. limits.timeout_seconds (default 60.0, documented in docs/diagnostics/CONFIGURATION.md) is never enforced.
The deadline check runs between rules. It does not stop a single long-running rule.

Record rules that return NOT_APPLICABLE from evaluate() as skipped.
Some rules decide applicability inside evaluate(). FkIndexCoverageRule returns NOT_APPLICABLE on MySQL and MariaDB. CheckConstraintMetadataRule returns NOT_APPLICABLE for engines it treats as unsupported.
These rules stay in evaluated_rule_ids and in total_rules_evaluated. They are never added to skipped_rule_ids. The coverage and summary then report the InnoDB-skipped FK rule as evaluated.

Sensitive Data Exposure
Reachability: Internal Exploitability: Difficult CWE:CWE-532 — Insertion of Sensitive Information into Log File
Fix the connection-string regex: it leaks part of any password that contains @.
_CONNECTION_STRING_PATTERN uses [^@]+ for the password. The match stops at the first @.
For postgresql://dbuser:s3cr3t_p@ss!@db.internal:5432/production, the output is postgresql://***:***@ss!@db.internal:5432/production. The password fragment ss! stays in the evidence.
test_evidence_sanitization still passes because it checks only that the full password string is absent.
The violated property is credential confidentiality in evidence that goes into reports and API responses. Match up to the last @ before the host/path boundary.

list_rules() passes every rule to the evaluator, which does not apply enabled_rule_ids, disabled_rule_ids, or category filters. A configuration that selects only foundational rules will therefore also run the nine newly registered rules. Filter the registry results using the supplied configuration before evaluation. Filter the rules returned by DiagnosticRegistry.list_rules() using the supplied
configuration’s enabled_rule_ids, disabled_rule_ids, and category filters before
passing them to the evaluator. Preserve the configured selection so unselected
rules are not evaluated.
Pass the schema capability profile to diagnostics.

Pass the schema capability profile to diagnostics.
The new rules run with capabilities=None, even when schema.capability_profile records unsupported or restricted metadata. For example, missing index metadata can produce an unindexed-foreign-key finding instead of an insufficient-metadata result. Pass the profile and adapt applicability checks to its features API; the current check calls a nonexistent get_state method. (raw.githubusercontent.com)

Pass schema.capability_profile into the DiagnosticContext created in the
reverse-engineering service so diagnostic rules receive the schema’s capability
information. Update applicability checks to use the capability profile’s
features API instead of the nonexistent get_state method.
