fix: clarify ladder button readiness failure

Summary

• extend the bounded native action-button readiness deadline from two to three seconds
• report confirmed ladder progress before the failure reason
• replace the ambiguous processing message with the exact readiness deadline outcome
• cover the approved final status text and timeout contract
