Convert large attachments when no bucket is set - #1332
Bedrock Converse and Gemini on Vertex AI upload chat attachments over their inline limits (4.5 MB on Bedrock, 7 MB on Vertex AI) to S3 or Cloud Storage when auto_upload_large_files is on. Without bedrock_batch_s3_uri or vertexai_batch_gcs_uri, a chat that attaches a large PDF fails mid-turn with a ConfigurationError raised from inside the upload (Protocols::Bedrock::Files#storage_uri_for, reached through preprocess_attachment and StoredUploads#fetch).
An application that registered convert_unsupported_attachments to turn files into text can't recover either: the handler only sees attachments whose format the protocol can't render, and a PDF is supported.
Solution
Protocols::Files#ensure_storage! raises when an upload without a uri: has nowhere to go. It is a no-op by default; the Bedrock and Vertex AI files protocols raise their existing "Set ... to a bucket prefix" error, and their upload path now calls the same method. Provider#ensure_file_storage! asks the provider's files protocol.
Before uploading a large attachment, Protocol#preprocess_attachment checks the storage. Without it, the attachment goes to the chat's converter, with the same validation as for unsupported formats. A replacement takes the attachment's place; without a converter, or when it returns nil, the storage error is raised before any request or upload.
Configured buckets, and providers with their own file APIs (OpenAI, Anthropic, Gemini API, ...), upload exactly as before.
The convert_unsupported_attachments RDoc and the attachments and files guides say what happens without storage.
Tests
spec/ruby_llm/protocol_spec.rb: for Bedrock Converse and Gemini on Vertex AI without a bucket, the handler's replacement is used and nothing is uploaded; without a handler, or with one returning nil, the storage error names the setting and nothing is uploaded; a non-attachment return is refused; with a bucket set, the attachment uploads. Anthropic still uploads without asking the handler. The new examples fail on main.
bundle exec rspec --tag ~live --tag ~generator: 6034 examples, 0 failures. RuboCop, Archspec, Flay, and the mutation pre-push hook pass.

Whether Claude on Bedrock Converse accepts S3 document locations at all wasn't checked against the live API: AWS documents S3 media links mainly for Amazon Nova. This change doesn't alter what is sent when a bucket is configured.

Bug Fixes
Large attachments can now be converted when a provider has no storage bucket configured, rather than failing immediately.
If conversion is unavailable or does not provide a replacement, requests now report the missing storage configuration before sending.
Providers with configured storage continue to upload large attachments as before.
Documentation
Clarified how attachment conversion, storage requirements, and model input limits apply to large attachments.
Bedrock Converse and Gemini on Vertex AI upload attachments over their
inline limits to S3 or Cloud Storage. Without bedrock_batch_s3_uri or
vertexai_batch_gcs_uri, a chat with a large PDF failed mid-turn from
deep inside the upload, and the chat's convert_unsupported_attachments
handler never saw the file because the format itself is supported.
The files protocol now says whether uploads have somewhere to go. When
they don't, preprocessing hands the attachment to the chat's converter,
and without a replacement raises the storage ConfigurationError before
any request or upload. Configured buckets upload as before.
Large attachments that require provider storage now use the attachment converter when storage is not configured. If conversion returns no replacement, preprocessing raises the storage configuration error. The changes also update documentation and tests.
Files protocols expose ensure_storage!. Bedrock and Vertex AI raise ConfigurationError when their storage settings are empty. Provider delegates to its registered files protocol.

Attachment preprocessing tries conversion when storage validation raises ConfigurationError and re-raises the error if conversion returns nil. Tests cover storage and conversion cases across providers. The documentation describes the fallback.

A converter that returns an attachment still too large for inline use can cause the request to fail at the provider. This is a bounded risk; return a smaller attachment or configure storage.

Explanation
The title "Convert large attachments when no bucket is set" matches the main change: when Bedrock Converse or Gemini on Vertex AI has no upload storage configured, large attachments are routed to the chat's converter instead of failing. It is specific, concise, and single-sentence.
Full details: Description check
Explanation
The description follows the template well. It has a clear problem statement and solution, and it covers tests run and what was not verified. It describes the change and why it was made. The template checkboxes are not visibly ticked, and the Related issue section is optional and absent. The description says documentation was updated, and the raw summary confirms the attachments, files, and chat RDoc changes. The description does not mention the API changes section, and it lists no breaking change, although the change adds a new public method (ensure_storage!) and Provider#ensure_file_storage!. The author should tick the template boxes and mark 'New public methods' under API changes.
Resolution
Optionally tick the template checkboxes (Type of change: Bug fix or New feature, Quality check, Ownership) and mark 'New public methods/classes' under API changes, since Protocols::Files#ensure_storage!, the Bedrock and Vertex AI ensure_storage! methods, and Provider#ensure_file_storage! are new public methods.

This reverts commit b1233f5.
It handed an attachment too large to send inline to the app's
convert_unsupported_attachments handler, which can't tell that from an
unsupported format, so an app converted a file it meant to send.
Without a bucket, a large attachment again fails before any request,
with the error naming the setting to configure.
