perf(reports): remove codec processes from collection

🤔 Background

Related #1371

Parallel reports launch codec processes for each row's text fields. Collect complete worker records without those processes while preserving public formats and completion order.

💡 Changes

• Publish separate lossless worker records and replay them once in completion order, including parent and file hook failures.
• Preserve arbitrary text and large output, avoid record collisions, and clean all records with the run.
• Cover concurrent publication, incomplete records, lifecycle failures, all report formats and zero codec calls. Update the changelog.
• Source, standalone, static and formatting gates pass. An isolated 100-row collector takes 46 ms instead of 1,185 ms and removes 400 base64 plus 200 tr calls; 512 KiB output takes 99 ms instead of 215 ms. Medians use five alternating pairs against main at f33a9ac on macOS Bash 3.2.
