OpenHealthOS is an early-stage platform initiative focused on building a secure, modular, and interoperable foundation for health data services, intelligent workflows, and extensible integrations.

OpenHealthOS aims to provide a flexible platform for health-focused applications that can support secure data exchange, modern service architecture, and future AI-assisted capabilities while remaining adaptable to evolving healthcare needs.

The mission of OpenHealthOS is to create a trustworthy and developer-friendly foundation for building health technology solutions with strong architecture, clear engineering practices, and long-term maintainability.

Architecture (High Level)

The platform is organized around a modular architecture that includes:

  • a gateway layer for ingress and routing
  • shared building blocks for contracts, infrastructure, observability, security, and common utilities
  • domain-oriented services for capabilities such as identity, patient workflows, FHIR integration, AI, audit, and notification
  • plugin-based extension points for optional integrations and custom behavior

Tech Stack

The initial technical direction emphasizes:

  • .NET 10 and ASP.NET Core for backend services
  • C# as the primary implementation language
  • Clean Architecture for modularity and maintainability
  • Microservices for independently evolving domain capabilities
  • Azure-first deployment with a cloud-agnostic design posture where practical

Roadmap

Planned early work includes:

  • establishing the repository and documentation foundation
  • defining architecture decision records and engineering guidance
  • organizing the source tree into domain-oriented modules
  • expanding service and platform documentation over time

Contributing

Contributions are welcome as the project evolves. At this stage, the contribution process is still being formalized, and contributors should expect to work collaboratively with the maintainers as the project matures.
