An escalation burst is sent once per identity line, read from the bus - #2574

The heartbeat's record of enforced rules lives in its process, so every restart (each install, and the store outage at 22:20 UTC) made every vibe-check sign and the chokehold read as new and resent their bursts: about 28 vibe-check lines at seq 16691-16725 within minutes of the repair. EscalationSender.burst_once skips a burst whose identity line its sender already sent the commander in the horizon: the vibe check's opening line names its sign (Sign.mark), the chokehold's names the ignored check, a refusal names the answer it refuses. P433 (#2469) still owns durable memory for every rule; this stops the escalations, which are the ones that pause the fleet. The heartbeat keeps its record of enforced rules in process memory. Every restart (each install, and the todo-store outage around 22:20 UTC) therefore made every vibe-check sign and the chokehold look new, and their bursts went out again: about 28 vibe-check lines at seq 16691–16725, within minutes of the repair.
EscalationSender.burst_onceskips a burst when its sender already sent the commander that burst's identity line within the horizon. Each burst's identity line:Sign.mark: the signal and its key);Ignored: vibe check SEQ:ID;That's not a quote: SEQ:ID.The chokehold still pauses the units every time it is enforced; that part is idempotent. The new test runs the same vibe check twice and gets one burst; a second sign gets its own.
P433 (#2469) still owns durable memory for every rule. This PR covers only the escalations, because they're the ones that pause the fleet.
