Review is per PR head and gates merging (phase 4). The heartbeat beats every pass as phase 7 already said, failures are bus facts, alarms declare what clears them, forks are registered threads with one assignment lock, and every rule names its mechanism (phase 7). The skill's Monitor setup goes (phase 9). Replaces the duplicate plan in the closed #2482.
