fix: update preset issue status comments

Description

• Post validation, edit, and approval results through one bot-owned issue status comment. Later runs update the existing comment instead of adding another.
• Reuse older unmarked bot status comments when a request is checked again.
• Add tests for validation followed by approval, an issue edit followed by another check, and unchanged results.
• Verified with npm test (33 passing), npm run lint, and git diff --check.
