Correct REVISION 6 evidence and historical wording

What changed

• correct the historical CI tense in REVISION 6
• replace stale REVISION 5 gauntlet counts and source binding in the Evidence Report
• narrow the negative-control claim to the failure scenarios actually exercised
• clarify that the old REVISION 5 binding is historical and superseded

Why

The first REVISION 6 evidence rebind updated the headline but left stale test counts and source-binding values in the gauntlet table. A historical note also used present tense, making an obsolete binding look current.

Validation

• full gauntlet passed after the source-scope edit and again after the evidence-only commits
• 9/9 source-state tests passed
• 50/50 total tests passed with 100% statement and branch coverage
• 22/22 mutants killed
• randomized 50-test suite passed 10/10 consecutive runs
