UI fixes and improvements

• When the user clicks on the "Coverage Map" tab, the page automatically jumps back to the top of the page. This behaviour should be removed, as it does not occur when clicking on any of the other tabs.
• Add tooltips
• Make the Question Plan table responsive to narrow screens (eg, decrease width of Count column, etc)
• In "Generate Questions" -> "Question Plan", increase the width of the text box for the Documentation Tool question type
• On the “learning objectives” and “review & edit” pages, make the “regenerate” icons consistent (ie, change the “refresh” icon to the same icon as “regenerate all”):

• Make UI in "Generate Questions" tab (both the "Generate Questions" and "Generated Questions" pages) consistent with the UI in the other tabs
