docs: note the cleaner agent timeline in the v1.0.4 release notes
PR #61 (thread the timeline rail + fold an approved MCP call into one row) merged after the v1.0.4 notes were first written. Add a 'A cleaner agent timeline' highlight, an intro mention, and a coverage bullet for the webviewCss invariants that guard it. The chat-sessions design doc (PR #60) is intentionally excluded — it is a design document, not a shipped feature. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

And an **approved MCP tool call is now a single row** — not an `Approved · server · tool` chip *plus* a separate `🔌 server · tool` node — so it's one row per action, matching how allow-listed calls already looked.
